YAP provides support to adults and their families with intensive support in their homes, work, and community at times most needed. Guided by their individualized plan, support can include a number of activities, including case management, crisis intervention services available 24/7, skill development, work on vocation and employment, and more.

YAP has successfully worked with families since 1975, but we have had specific programs for adults within the justice system and with developmental disabilities. We have further developed program models to serve adults with substance abuse and other health challenges.

As in our youth services, we have tailor our approach to better meet the specific needs of adults through our experience and best practice research. As a result, we have developed a number of adaptations and specializations in our model based on the needs that we have identified.
