Families In Transition

FIT When a family is in crisis and needing support services, one size does NOT fit all.

Families in Transition (FIT) Program provides specialized services to meet the individual needs of youth referred from the Tarrant County Juvenile Justice System.FIT provides family centered services needed to address youth who have substance abuse issues, family conflict, life skills deficits, or in a situational crisis.

FIT Program includes a combination of case management and counseling services with the goal of stabilizing youth in their community and providing skills to successfully complete probation requirements. All youth admitted into the FIT program have a Care Coordinator who works with the youth/family to determine their needs and create a “care plan” to address identified challenges. That assessment will determine which of the following counseling services will best meet the needs of the youth and family:

  • Family counseling
  • Trauma counseling
  • Individual drug counseling
  • Drug aftercare services
  • Cognitive Behavioral Therapy

The Care Coordinator remains with the family from time of admission until discharge and:

  • Assist families in finding ways to overcome any barriers to program completion
  • Makes referrals and connects families with other needed services
  • Helps participants engage in community services and /or support group(s)

FIT services are provided for youth referred from the Tarrant County Juvenile Services and are offered in English and Spanish.
